In some cases you can just cut a hole in the side panel, mount a quiet fan there, and finish off the cut metal edges with automotive door edge trim from an auto parts store ~ $2.
At least then, unlike a typical small video card fan, if it ever fails you can source a standard replacement fan.
I do have a problem with tiny fans on video cards, in that I end up having to pull them at some point in their life and re-lube or find some way to replace them with a fan mounted elsewhere, or at the very least to have a backup plan like motherboards with integrated video too or having one or more spare video cards lying around.
Unfortunately the poor airflow condition present in many cases also causes excessive heat on integrated video, though moreso last generation(s) with the IGP integrated into a passively cooled northbridge rather than CPU.

Not everyone needs anything faster.
No fan = less noise which is a good thing, not to mention less moving parts to fail.
