2 cooling fans means it's most likely inefficient.

#2, not necessarily. Some companies use two fans running at slower speeds to reduce noise, but still have the same air flow.

Maybe it's just a crappy power supply. There are a lot of them out there, and it's amazing that people are willing to put their whole system at risk to try to save $20 on a power supply.
