Your homeowner's or renter's insurance contract may already include a rider that covers your iPhone, iPad, and other gadgets against theft, breakage, etc...
Check with your insurer before you spend money you don't have to. Different insurance companies have different requirements for coverage.
Really? You can sometimes negotiate an early renewal, get basic screen cracks fixed by a bunch of people, etc. You might already be covered by renter's insurance. If all that fails, you can buy used ones on ebay/craigslist, etc for not much more than this.
If all that even fails, you'll usually be lucky as sin to get these guys to cough up the cash to replace/fix it. They'll likely take days to weeks before you'll get a refurb or replacement or fix. That's all the time you're without your phone.
In other words - worthless deal.
