#1, you need to contact AZiO Rebates <rebate@aziocorp.com> (read the terms and conditions section) and request for the rebate status providing them with your particulars.
I also had a rebate pending since last Nov like you and I got a prompt reply to my email and the following week a cheque. Good Luck.
