#1, rebate form doesn't limit to online purchase, if you want you could buy at the B&M and use the rebate Ben linked.

#4, For the fry's rebate, I did 3-4 rebates with Fry's about 4 years ago and never gotten anything back. So I never buy anything from them that requires rebates. Maybe things changed by now. Good luck!
