Because in order to serve a specific territory, they would have to ensure they could provide 100% of customers with the fiber-optics... Not only is it expensive to run, its a gamble in smaller areas if they don't get enough subscriptions
Small markets, heck I'm here in Silicon Valley and can't get FiOS or anything else (non-Comcast) -- so we're sitting here in so-called Tech Wonderland being raped on fees by Comcrap. And even more ironic, my town is an old GTE stronghold. (For younger readers: GTE beget Verizon) Wish Verizon would spend more money laying fiber and less on saturation advertising (not to mention frequent "we miss you" mailings to re-sign Verizon landline). But seeing as how the US telco oligopoly has carved up the market into non-competitive chunks, I am not holding my breathe.
If I were in Silicon Valley I'd be more apt to blame the lack of fiber deployments on regulations related to seismic concerns, greenbelt restrictions, and the NIMBY attitudes toward distribution cabinets.

Too bad, no love here in SF/Bay area, just scumcast and their dumb techs and greedy sales
#3, be careful what you wish for. I use Comcast for cable and Verizon for DSL. IMO they both suck. Verizon even screwed up the wiring in my house.
